Initial Psychiatric Evaluation

A comprehensive 60-minute assessment to understand your mental health history, current symptoms, and treatment goals. This visit includes diagnosis, treatment planning, and discussion of medication options if appropriate.
Crisis or Urgent? **Guaranteed Same-Day Visits Available

Guaranteed same-day appointment for patients experiencing an acute increase in symptoms or needing timely medication adjustments. Duration up to 30 minutes.
Follow-Up Medication Management
$170
A 25–30 minute visit to monitor progress, manage medications, address side effects, and make adjustments to your treatment plan as needed. This is a routine check-in to ensure your care stays on track.
Late Cancellation / No-Show Fee
$75
Fee applied if an appointment is missed or canceled with less than 24 hours’ notice.
Documentation & Administrative Fees
We’re happy to provide documentation when clinically appropriate. The fees below reflect the time, effort, and liability involved in completing various types of requests. We charge a fee due to the time and liability involved.--
Basic Letters and Forms
$25
This includes brief, non-complex documentation such as general mental health status letters, provider-to-provider communications, or short statements in support of temporary accommodations.
There is no charge for simple school or work excuse notes or proof of attendance.
Forms Requiring Clinical Judgment
$55
Completion of complex forms that involve clinical decision-making, such as FMLA, disability paperwork, emotional support animal (ESA) letters, or accommodations for school or work.
These require provider review, documentation, and are not guaranteed to be approved. These forms are completed at the provider’s discretion and may require an in-person visit or additional assessment. Please allow 5–7 business days for processing.
Please note: All prices listed are for self-pay (out-of-pocket) clients and do not apply to insurance-covered visits. Fees are subject to change and will be confirmed at the time of scheduling.
